Medication Therapy Management (MTM) is a free program offered as part of Medicare Part D plans to certain members that meet specific criteria, as defined by the Center for Medicare and Medicaid Services (CMS). The MTM program is provided to help improve patients' understanding of their medication and its usage to manage their chronic conditions better (for example, diabetes, hypertension, asthma) and improve their overall health.
